Tracy Polacheck is the Executive Director of Pathstones by Phoebe where she is responsible for the overall operations of the Continuing Care at Home program and oversees the development and implementation of comprehensive programs and service offerings to help members stay well and maintain their highest level of independence. Most recently, Polacheck served as Senior Executive Director at Keystone Villa Communities in Ephrata where she was responsible for overseeing the operations of Independent, Personal, and Memory Care. With nearly 20 years’ experience, she has spent the majority of her career opening and managing a wide spectrum of retirement communities in Berks and Lancaster Counties. She is a Licensed Administrator for both Personal Care and Assisted Living. Polacheck graduated from Penn State University with a Bachelor’s Degree in Kinesiology with a concentration in gerontology.
Cynthia (Cindy) Moser
Cynthia (Cindy) Moser serves as the Membership Coordinator for Pathstones by Phoebe. She earned her Bachelor degree from Cedar Crest College in Business Administration with a focus on marketing. Cindy is also an Alzheimer’s Foundation of America Certified Dementia Care Partner. Coming from Lehigh University as a coordinator, Cindy brings solid program support and management skills. For 13 years prior, she owned and operated Just My Style Promotions, a small business she created offering embroidery and promotional products to area businesses, teams, and individuals.
Amanda Longey, MS, ACE Certified Health Coach
Amanda Longey serves as Wellness Coordinator of Pathstones by Phoebe. She earned her Bachelor of Science in Psychology from Mansfield University and her Master of Science in Gerontology from McDaniel College. She is an ACE Certified Health Coach and Senior Fitness Specialist. She is also a Certified Dementia Care Partner from the Alzheimer’s Foundation of America. Amanda is a professional member of the Aging Life Care Association™. Amanda has over 11 years of aging services experience specializing in care management and wellness programming. Amanda brings a wealth of knowledge of long-term care and a passion to help the aging population stay healthy and independent.
