Tracy Polacheck is the Executive Director of Pathstones by Phoebe where she is responsible for the overall operations of the Continuing Care at Home program and oversees the development and implementation of comprehensive programs and service offerings to help members stay well and maintain their highest level of independence. Most recently, Polacheck served as Senior Executive Director at Keystone Villa Communities in Ephrata where she was responsible for overseeing the operations of Independent, Personal, and Memory Care. With nearly 20 years’ experience, she has spent the majority of her career opening and managing a wide spectrum of retirement communities in Berks and Lancaster Counties. She is a Licensed Administrator for both Personal Care and Assisted Living. Polacheck graduated from Penn State University with a Bachelor’s Degree in Kinesiology with a concentration in gerontology.
Cynthia (Cindy) Moser
Cynthia (Cindy) Moser serves as the Membership Coordinator for Pathstones by Phoebe. She earned her Bachelor degree from Cedar Crest College in Business Administration with a focus on marketing. Cindy is also an Alzheimer’s Foundation of America Certified Dementia Care Partner. Coming from Lehigh University as a coordinator, Cindy brings solid program support and management skills. For 13 years prior, she owned and operated Just My Style Promotions, a small business she created offering embroidery and promotional products to area businesses, teams, and individuals.
Mimi Leinbach is the Wellness Coordinator for Pathstones by Phoebe. She earned her Bachelor’s degree in Psychology from Lock Haven University concentrated on counseling, which enabled Mimi to begin her career helping adults with mental illness. Previously, Mimi was a supervisor for a residential service provider, developing comprehensive knowledge of hospital case management, which is vital to her role at Phoebe. Mimi also enjoys volunteering at her children’s schools, and has been published multiple times for her creativity in paper arts.
