Various plans and options are available:
- Platinum and Diamond/Estate Refund Plans: In-Home Care Limit – $12,000/month
- Gold Plan: In-Home Care Limit – $9,000/month
- Silver Plan: In Home Care Limit – $6,000/month
- Bronze Plan: Care Coordination Only – other person in household must have one of above plans
- Sterling Plan: Care Coordination Only for individuals with long-term care insurance
- Early Advantage Plan: Care coordination only for individuals under the age of seventy-six

The one-time membership fee is based on age at time of membership and plan selected.
The monthly fee is based on the plan selected.
*5% discount available for persons living in the same household
