What is Continuing Care at Home?
Pathstones by Phoebe is a Continuing Care at Home (CCaH) program. What does this mean? In short, a CCaH program is a membership program that provides a wellness coordinator who advises you on living a healthy, active lifestyle. Pathstones combines the key features of a CCRC, such as valuable long-term care management and priority access to health care services. Additionally, Pathstones covers the cost of your care, thereby alleviating any worries about depleting all of your hard-earned financial resources to pay for care. With Pathstones, you remain in your home, wherever you call home. In practical terms, a program like Pathstones helps you live a healthier life today—including social, cultural and fitness activities and programs—while simultaneously securing your future.
What does a wellness coordinator do?
Our qualified wellness coordinators will collaborate with you to develop a personalized wellness plan to maintain your overall health in your home. The aspects of this plan include your physical, social, emotional, environmental, intellectual, spiritual and vocational well-being. Wellness coordinators lead quarterly check-ins to ensure you are continuing to live your best life possible. As a member of the Pathstones family, your wellness coordinator is a partner for life.
What are the costs of the Pathstones program?
Each level of Pathstones membership offers different payment points to best serve your individual and financial needs. Each plan includes a one-time membership fee based on your current age and the plan you selected. In addition, each plan requires a monthly fee that is based on the plan you selected. Plan eligibility is determined by financial and medical evaluations. For example, a 65-year-old who selected the Platinum Plan may have a one-time membership fee of $57,606 and a monthly fee of $500. Discounts are provided for two or more individuals living in the same household. Additionally, discounts are offered for individuals have long term care insurance. What types of plans are offered?
We offer five plans with daily benefits that start at $100 per day and go up to $410/day. This means that up to $410/day can be covered for various health services. These services include Home Health Coverage, Home Care Services, Live-in Companion Services, Adult Day Services, Personal Care/Assisted Living Services and Skilled Nursing Care. These plans offer various levels of care and different types of services based on how your needs evolve in the future. For example, our Diamond Plan offers full coverage of all services and an estate-refundable entrance fee. Whether you just need a wellness coordinator to meet your goals or you need care as involved as skilled nursing, all of it will be covered. Meanwhile, our most affordable Sterling Plan covers up to $100 per day for services provided while you remain in your home. Who is eligible to enroll in Pathstones by Phoebe?
Any healthy person 55 or older living in Lehigh, Northampton, Bucks or Berks County, as well as any surrounding counties in Pennsylvania, is eligible to sign up for Pathstones by Phoebe.
What if my needs change?
That’s what our wellness coordinators are for! Our wellness coordinators will help you prepare as your health care needs change. If your needs change and you no longer can remain in your home, our wellness coordinators will help you find the right place to provide you with appropriate levels of care. Your wellness coordinator serves as your advocate to facilitate the best path for your care and determine next steps. And if you should move or have a winter or summer home anywhere else in the continental US, we will coordinate your care there as well.
