Pathstones by Phoebe is a continuing care at home (CCAH) program designed to provide life-long security and protection for your ongoing needs while you remain in your home. It is a membership-based program that uniquely combines the most important features of long term care into one comprehensive plan. The goal of Pathstones is to help adults live in their homes even as their needs change. We work with members before health and wellness needs emerge, providing proactive care that limits the need for medical services later.
What are the two newest plans offered by Pathstones?

Who is eligible to enroll in Pathstones by Phoebe?
You are eligible to apply for Pathstones membership if you are a healthy person, age 55 or older, and have a residence in Pennsylvania. Please contact us to learn about the member application process.
How does Pathstones protect my assets?
Pathstones provides a lifetime benefit for care costs with no maximum limits. This alleviates the concern of using all of one’s savings and assets to pay for long-term care. Assets are protected without the need to set up any type of trust arrangements, which depend on medical assistance to pay for care and limit options for quality care providers.
How does the fee structure work?
A Pathstones membership consists of a one-time membership fee based on your age at the time of enrollment and monthly fees throughout your membership. Fees depend on the plan you select.
What is the average age people join Pathstones?
The average age at the time of membership is 74. The minimum age for membership is 55, and while there is no maximum, an individual must be in good health and deemed to be not at risk of needing services for a minimum of 2 years.
What if I have long-term care insurance?
Long-term care insurance is only one part of long-term care planning, while Pathstones is a complete plan that includes care coordination and advocacy. If you have long-term care insurance, Pathstones will review your plan and discuss gaps that can be filled by combining Pathstones with your long-term care insurance.
Do I really need care coordination?
Studies have shown that Care Coordination results in fewer emergency room visits, fewer hospital readmissions, improved quality of life, and later mortality.
Doesn’t Medicare pay for long-term care in a skilled nursing facility?
Medicare and Medicare Advantage plans pay for care in a skilled nursing facility only when care requirements are met. The average length of stay varies between 20-38 days. Medicare and Medicare Advantage plans do not pay the daily rate for non-rehabilitative stays. In 2020, the average cost for a semi-private room at a nursing facility in Pennsylvania was $10,038 per month.*
