Six plans options are available:

• Diamond: 50% of Membership fee is refunded to Estate.
  Pays 100% of Daily Benefit Limit.
• Platinum: Pays 100% of Daily Benefit Limit.
• Gold: Pays 75% of Daily Benefit Limit.
• Silver: Pays 50% of Daily Benefit Limit.
• Sterling: Care Coordination only for individuals with long-term care insurance.
• Early-Advantage: Care Coordination only for individuals under the age of 76.

As of 07/01/2022: Daily Benefit Limit – $420/day

The one-time membership fee is based on your age and plan selected.

The monthly fee is based on the plan selected.

Example: Silver Plan – Age 74

• One-time membership fee – $40,459*
• Monthly fee – $470*

*5% discount available for persons living in the same household
